To Whom It May Concern, I am submitting this formal complaint regarding the traumatic and unacceptable treatment my elderly mother experienced under the care of your facility and medical staff. This complaint concerns serious issues of patient neglect, unprofessional conduct, and lack of compassion, which resulted in physical injury and prolonged hospitalization. On the date of care, the anesthesiology team—identified as Reuben Uniat and Tiffany Leake—attempted to intubate my mother. During this process, significant damage was caused to her throat. As a direct result of this incident, my mother was unable to eat for four days and ultimately required hospitalization for a total of five days. This outcome represents a serious failure in patient care, particularly given my mother’s age and vulnerability. Following this incident, Bradley Litzkow, who identified himself as the Facility Administrator, acted in an unprofessional and dismissive manner. He was rude, refused to provide discharge paperwork when requested, and failed to address our concerns appropriately. This refusal obstructed our ability to understand my mother’s condition, aftercare needs, and the events that occurred during her treatment. Additionally, Dr. Manaf Younis and the medical team involved demonstrated a complete lack of compassion and accountability. At no point did the family feel heard, supported, or treated with the dignity and respect that patients and their families deserve. The conduct displayed fell well below acceptable professional and ethical standards and constitutes what I believe to be neglect of an elderly patient. I am holding all involved parties accountable for my mother’s traumatic experience. This complaint is submitted with the expectation of a full investigation into: • The actions of the anesthesiology team during intubation • The resulting injury and subsequent hospitalization • The refusal to provide discharge documentation • The conduct and oversight of the facility administrator • The overall failure to provide compassionate, safe, and appropriate care I request written acknowledgment of this complaint, a detailed explanation of the findings of your investigation, and information regarding any corrective or disciplinary actions taken. My mother’s experience has caused unnecessary physical harm, emotional distress, and suffering to our entire family. No patient—especially an elderly one—should ever be treated in this manner. My husband had to have a spinal simulator put in his back anytime he has surgery I get nervous but the staff and particularly Neala in the front made me feel comfortable and got me coffee! And the discussion about or kids made the time fly! We definitely recommend thank you Neala!
